H1N1 FLU VACCINATION CLINIC

Available for all children 6 months through 18 years old, pregnant women, and parents with children under 6 months of age

Vaccinations will be held at the community field clinics in

Parshall- Wednesday, November 4, 9:00am - 3:00pm

Mandaree- Thursday, November 5, 9:00am – 3:00pm

White Shield- Friday, November 6, 9:00am – 3:00pm

Twin Buttes- Monday, November 9, 10:00am – 3:00pm

And at the Community Health Building in the North East Corner of the Minne-Tohe Health Center in

New Town- Tuesday, November 10, 9:00am – 3:00pm

There is a very limited supply of vaccine, vaccination clinics are subject to cancellation or vaccine running out.

Vaccination available on a walk in basis, first come first serve. No appointments. Please allow for possible waiting time.

Please have your child’s vaccination record for the past 30 days available.

As more vaccine becomes available over the next several weeks we will begin offering additional vaccination clinics.

Vaccine forms can be picked up from your local CHR or at your local field clinic.

Please do not come to vaccination clinic with any flu like symptoms.
