Demographic Information needed:
- Name and DOB
- MHA enrolled member
- Verify Insurance
- Phone number
- Drug of Choice (DOC) and last use
Treatment placement priority’s:
- Pregnant women
- IV and fentanyl users
- Opioid and alcohol users
- Incarcerated clients
Step 3
Our Team will work with every individual who needs services to ensure you receive the services you need by:
- Gathering Consents: Release of Information and other documentation necessary.
- Contacting treatment facilities for availability and placement.
- Coordinating for evaluations, physicals, COVID Testing, and any other requirements for admission and/or placement.
- Coordinating Transportation services.

Good Road Recovery Center provides addiction services based on the American Society of Addiction Medicine (ASAM) level of care:
- Outpatient
- Level 1 (Aftercare, Individual sessions)
- Level 2.1 (Intensive Outpatient)
- Level 2.5 (Partial Hospitalization)
- Residential
- Level 3.1 (Low Intensity Residential)
- Level 3.2 (Social Detox)
- Level 3.5 (High Intensity Residential)
- Adolescent Services
- Level 2.1 (Intensive Outpatient)
- Level 1.0 (Aftercare, Individual sessions)
- Intensive Case Management
- Peer SupportMental Health Assessments/Therapy
Admission Criteria
We admit, Adults (18 years of age & older) and Adolescents (17 years old and younger) who meet criteria for substance use disorder.
*Currently, Outpatient adolescent services are only available for those that qualify in the New Town are.
