Are You Aware of The Be Well Benefit?
Are you enrolled in Critical Illness Insurance or Hospital Insurance with UNUM under your employee benefits with the Tribe? If so, you are eligible for a $50 incentive called the Be Well benefit. This benefit pays $50 to the employee or their dependents under the employee’s critical illness or hospital insurance plans if they completed a Be Well screening such as an annual exam, cancer screenings, or even immunizations!
Employees who are enrolled in critical illness and hospital insurances are still eligible to submit claims that happened in 2022 if they have not filed a claim.
Filing a claim is easy! You can call 1-800-635-5597 or file online at unum.com. We are also available to help you if you need assistance or have any questions. Email us at Employee.Benefits@mhanation.com or visit us at our office located in the old Tribal Administration Building, 404 Frontage Road, New Town.
