Program Overview
The IHS Community Health Representative (CHR) Program provides guidance and support to a national Tribal Community Health Workforce identified as Community Health Representatives (CHRs) working across American Indian and Alaskan Native (AI/AN) communities. CHRs are frontline public health workers who improve access to healthcare in AI/AN communities and build community capacity.

Who We Serve
Must be Enrolled in a Federally Recognized Tribe & reside within the boundaries of the Fort Berthold Indian Reservation. *Priority of services are given to: Elders, Cancer Care Clients, Dialysis Clients, & Homebound individuals. Community Health Representatives are CPR/First Aid Certified and C.N.A. Certified. All CHRs attend trainings to maintain certifications.
Services Provided
CHRs Increase Access to Healthcare and Increase Coverage through the following activities:
Health Education & Promotion: Such as, healthy lifestyles, physical activity, basic nutrition, smoking cessation, seat belt use, drug and alcohol use, STIs, hand washing, infection control, etc
Case Management: CHR assists clientele in developing a plan that coordinates and integrates support services that the patient needs to achieve health care goals and outcomes.
Advocacy and Resource Navigation: Advocate for clients and assist clients in finding and accessing resources.
Patient care: Provide blood pressure checks, blood glucose checks, and limited wound care (changing and cleaning).
Homemaker Services: Services such as light cleaning, removal of trash, light cooking, meal planning, and shopping.
- Transportation: To/from non-emergency medical appointments. To schedule transportation, you will need to provide tribal enrollment verification and appointment confirmation.
You must schedule transportation Appointments at least 36 hours in advance.
All minors under the age of 18 must be accompanied by a parent or legal guardian.
Environmental Services: Contact programs to help connect clients/patients with services needed such as snow removal, trash removal, to clear a safe pathway to and from outside, etc.
Medication Delivery: Deliver medications and medical supplies to clients/patients’ homes.

Cribs for Kids Education: Cribs for Kids is a National Infant Safe Sleep Initiative. For more information about safe sleep for babies visit, www.cribsforkids.org.
Cribs for Kids® Program
Provides safe sleep education and a free crib for eligible families.
